2012 Funding (Year 3)
- Amount of funding: $1,093,662
- Description of Project
Awardee will:- Use collaborative processes that result in agency strategic plans, state/community health assessments, and state/community health improvement plans to meet accreditation prerequisites while identifying and addressing shared priority health issues
- Conduct quality improvement projects to increase agency efficiency, effectiveness, and customer satisfaction
- Collect and use performance management data to drive decision‐making and identify best practices
- Launch strategies and tools to develop, improve, support, and assure a competent public health workforce
2011 Funding (Year 2)
- Amount of funding: $1,093,662
- Description of Project
Awardee will:- Use performance management and quality improvement tools to prioritize activities, practices, and programs and to eliminate those with limited or no public health impact
- Integrate the identification and submission of promising practices with business operations for a sustainable approach to increasing the number of promising and best practices submitted and implemented
- Ensure that 70% (47 of 67) county health departments (CHDs) will produce at least 2 of the 3 prerequisite documents for national, voluntary agency accreditation
- Ensure that CHDs and state program partners will present at least four one-hour webinars on strategies to address cross-jurisdictional public health policy and law related-priorities using the Community Guide, other evidence-based practice compendia, and local best practice models
2010 Funding (Year 1)
Component I
- Amount of funding: $400,000
- Description of Project:
Awardee will:
- Focus on building, institutionalizing, and implementing performance management capacity within health departments
- Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
Component II
- Amount of funding: $1,660,128
- Description of Project:
Awardee will enhance public health improvement efforts beyond Component I through two or more categories including: performance management, policy and workforce development, public health system development/redevelopment and best practice implementation.
