2013 Funding (Year 4)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Prepare for a Public Health Accreditation Board site visit and achieve accredited status
    • Train at least 90% of senior leaders and managers on quality improvement (QI) basics and have at least 50% of non-managerial staff attend one of the nine QI courses given by agency trained staff
    • Complete, at minimum, two QI projects
    • Use the findings of the Performance Management (PM) Self-Assessment Report to strengthen the PM system, with a particular focus on improving the reporting of progress and QI activities

2012 Funding (Year 3)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Complete the three accreditation prerequisites by leveraging the work already done by categorical programs into a comprehensive planning process
    • Educate managers and staff about the performance management system and online performance system
    • Implement the new quality improvement (QI) plan to educate employees and foster a process where QI projects are initiated at the staff level
    • Decrease the rate of foodborne illness by completing a QI project to improve the quality of food establishment inspections
    • Decrease the time that staff spends responding to customer inquiries by completing a QI project to improve the effectiveness of communication materials for individuals seeking Board of Dentistry licensure

2011 Funding (Year 2)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Accelerate readiness to apply for accreditation
    • Conduct quality improvement initiatives
    • Create partnerships with organizations that have a proven record of performance improvement
    • Conduct a community health needs assessment
    • Implement one or more quality improvement projects within the agency

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
