2012 Funding (Year 3)

• Amount of funding: $250,000
• Description of Project
  Awardee will:
  • Complete the three accreditation prerequisites by leveraging the work already done by categorical programs into a comprehensive planning process
  • Educate managers and staff about the performance management system and online performance system
  • Implement the new quality improvement (QI) plan to educate employees and foster a process where QI projects are initiated at the staff level
  • Decrease the rate of foodborne illness by completing a QI project to improve the quality of food establishment inspections
  • Decrease the time that staff spends responding to customer inquiries by completing a QI project to improve the effectiveness of communication materials for individuals seeking Board of Dentistry licensure

2011 Funding (Year 2)

• Description of Project
  Awardee will:
  • Accelerate readiness to apply for accreditation
  • Conduct quality improvement initiatives
  • Create partnerships with organizations that have a proven record of performance improvement
  • Conduct a community health needs assessment
  • Implement one or more quality improvement projects within the agency

  • Focus on building, institutionalizing, and implementing performance management capacity within health departments
  • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
