2013 Funding (Year 4)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Finalize the state health improvement plan
    • Complete an organizational strategic plan informed by the statewide health assessment and health improvement plan
    • Engage in activities to address deficiencies identified in the Public Health Accreditation Board (PHAB) standard self-assessment
    • Complete the health reform strategy and associated initiatives and measures
    • Submit statement of intent to PHAB to apply for national accreditation

2012 Funding (Year 3)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Complete a statewide health improvement plan (SHIP)
    • Develop an agency-wide strategic plan informed by a health assessment and the SHIP
    • Conduct an organizational self-assessment to identify gaps in meeting and/or conforming with Public Health Accreditation Board standards
    • Develop and implement a health reform strategy and strategy map aimed at increasing access to quality and safe health care in Delaware
    • Complete a health equity strategy and map aimed at achieving health equity in Delaware
    • Increase tracking and performance reporting using a performance management system
    • Conduct two performance/quality improvement initiatives aimed at increasing efficiency and/or effectiveness

2011 Funding (Year 2)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Increase quality improvement training and consultation
    • Accelerate the implementation of accreditation activities
    • Increase capacity to implement a performance management and improvement system
    • Accelerate the use, implementation, and dissemination of evidence-based (best and promising practices) activities
    • Enhance information technology infrastructure to support performance management activities

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
