2013 Funding (Year 4)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Finalize and release the Live Well San Diego community health assessment and community health improvement plan
    • Apply to Public Health Accreditation Board (PHAB) for national public health accreditation
    • Complete an organizational self-assessment using PHAB’s Standards and Measures Version 1.0 and address identified gaps in order to improve ability to meet these standards
    • Conduct quality improvement in the areas of cultural and linguistically competent communications, health equity, and community engagement in the community health improvement planning process
    • Establish a performance management electronic system to manage the measures in its indicators framework

2012 Funding (Year 3)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Build and develop its accreditation team
    • Collect and review documents to submit to the Public Health Accreditation Board
    • Conduct quality improvement projects to improve organizational communication, health equity in programs and services, and workforce assessment and training
    • Share experiences, lessons learned, and best practices with other jurisdictions

2011 Funding (Year 2)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Increase accreditation readiness
    • Develop a performance improvement plan and implement quality improvement and performance improvement practices to ensure program efficiencies
    • Implement a comprehensive performance management system that links program performance to community health indicators
    • Engage in cross-jurisdictional partnerships to share strategies and practices
    • Develop a health equity plan that integrates with the performance management system as part of an increasing number of implemented promising and best practices

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
