• Description of Project
  Awardee will:
  • Complete accreditation prerequisites and apply for public health accreditation
  • Build performance improvement capacity by supporting performance improvement training and projects and working with external partners to increase effectiveness and efficiency of public health programs
  • Develop and implement performance improvement projects
  • Develop an agency-wide performance management system

2011 Funding (Year 2)

• Amount of funding: $300,000
• Description of Project
  Awardee will:
  • Establish a plan for developing a comprehensive public health data system for performance improvement management in two areas — electronic medical records  and the Environmental Health Data System
  • Increase capacity of key staff to lead quality improvement (QI) efforts and undertake projects to increase efficiencies of program operations
  • Increase the use of evidence-based policies and practices among cross-jurisdictional partners by hosting a conference for health professionals on health literacy and QI
  • Conduct a state health assessment, which will meet one of the prerequisites for accreditation

2010 Funding (Year 1)

  • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
