2012 Funding (Year 3)
- Amount of funding: $300,000
- Description of Project
Awardee will:- Complete accreditation prerequisites and apply for public health accreditation
- Build performance improvement capacity by supporting performance improvement training and projects and working with external partners to increase effectiveness and efficiency of public health programs
- Develop and implement performance improvement projects
- Develop an agency-wide performance management system
2011 Funding (Year 2)
- Amount of funding: $300,000
- Description of Project
Awardee will:- Establish a plan for developing a comprehensive public health data system for performance improvement management in two areas — electronic medical records and the Environmental Health Data System
- Increase capacity of key staff to lead quality improvement (QI) efforts and undertake projects to increase efficiencies of program operations
- Increase the use of evidence-based policies and practices among cross-jurisdictional partners by hosting a conference for health professionals on health literacy and QI
- Conduct a state health assessment, which will meet one of the prerequisites for accreditation
2010 Funding (Year 1)

- Description of Project:
Awardee will:
- Focus on building, institutionalizing, and implementing performance management capacity within health departments
- Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
