• Description of Project
  Awardee will:
  • Complete an accreditation readiness self-assessment
  • Move toward completion of a statewide health assessment
  • Collaborate with clinic and Disease Control staff to improve specific program protocol using quality improvement tools and methods
  • Complete a clinic efficiency project that was begun in 2012
  • Conduct staff training in quality improvement and performance management methods

2011 Funding (Year 2)

• Description of Project
  Awardee will:
  • Develop performance management capacity to improve the quality, effectiveness, and efficiency of the public health infrastructure
  • Accelerate current grant activities within the Office of Performance Management that address the need for quality improvement capacity to assure the systems supporting public health services and programs are efficient and effective

2010 Funding (Year 1)

  Awardee will:
  
  • Focus on building, institutionalizing, and implementing performance management capacity within health departments
  • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
