2012 Funding (Year 3)
- Amount of funding: $400,000
- Description of Project
Awardee will:- Release community health assessment and the 2012 state health improvement program progress report
- Continue to implement its state health improvement process
- Launch its strategic planning process and complete its strategic plan
- Address deficiencies identified in the organizational self-assessment, which measured gaps in meeting and/or conforming with national Public Health Accreditation Board (PHAB) standards
- Prepare a PHAB readiness plan, identify staff roles, assemble documentation, and submit PHAB application for accreditation
- Implement three quality improvement initiatives in performance management, asthma management in schools, and HIV and sexually transmitted infections clinical care guidelines at specific provider sites
- Contract with the Public Health Foundation to provide consultation in public health workforce development and to help create a Maryland public health workforce development plan that complies with PHAB standards
2011 Funding (Year 2)
- Amount of funding: $400,000
- Description of Project
Awardee will:- Continue development of a state health improvement plan (SHIP) through public engagement, measure refinement, and support of local action
- Provide resources and technical assistance to local communities for community health assessments and alignment of activites with the SHIP
- Develop an interactive dashboard to frame regional and local community health assessments
- Hold regional meetings across the state to bring together hospital community benefit personnel, local health department leadership, and community providers to encourage collaboration
- Identify a process for assessment of their public health system
- Develop an agency strategic plan
2010 Funding (Year 1)

- Description of Project:
Awardee will:
- Focus on building, institutionalizing, and implementing performance management capacity within health departments
- Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
