2013 Funding (Year 4)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Develop a Healthy Alaskans 2020 implementation plan/health improvement plan to achieve leading health indicator targets
    • Assess the statewide public health system to identify strengths and opportunities for Healthy Alaskans 2020 implementation and for all future health efforts
    • Reduce costs and eliminate waste in pharmaceutical ordering
    • Increase dissemination of public health data to the public
    • Align statewide efforts behind prioritized health goals in collaboration with the Alaska Native Tribal Health Consortium

2012 Funding (Year 3)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Complete a statewide health assessment
    • Actively participate in the state‚Äôs Health Information Exchange Meaningful Use clinical workgroup
    • Continue cross-jurisdictional collaborations with the Alaska Native Tribal Health Consortium and other stakeholders

2011 Funding (Year 2)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Support a full-time equivalent "Performance Improvement Manager"
    • Develop an agency-wide strategic plan
    • Begin planning for development of Healthy Alaskans 2020: Indicators and Targets for Improved Health
    • Train partners, stakeholders, and agency staff in performance management and strategic planning
    • Develop a state/tribal/community health assessment
    • Reduce health care costs through long-term improvements in health outcomes

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
