CDC Online Resource Directory Registration Form
The CDC Online Resource Directory (CORD) is a tool designed to increase communication between CDC and state, tribal, local, and territorial (STLT) public health agency staff. The goal is for CORD users to receive quick responses from CDC staff and contractors regarding public health issues. Users can search CDC employee and contractor contact information and access a phone number or send written communication through the system.
CORD is made available only to STLT public health employees who have registered for access.
Please fill out all of the fields below. If you have questions, please contact firstname.lastname@example.org or call 1-866-835-1861.
Reminder: CORD is made available only to state, tribal, local and territorial public health staff. If you are not part of this group but would like to contact CDC, please call 1-800-CDC-INFO (1-800-232-4636).
Disclaimer: For quality assurance purposes, the Office for State, Tribal, Local and Territorial Support receives a copy of any written communication sent through CORD.
