2013 Funding (Year 4)

  • Amount of funding: $399,987
  • Description of Project
    Awardee will:
    • Develop an agency-wide dashboard which measures programmatic performance by office as well as relevant financial, human resource, and operational data
    • Institute a new process for overall governance of operations which streamlines many of the current administrative processes
    • Complete automation of the bedding and upholstery permit and payment system as well as automation of the payment systems for the Offices of Environmental Health Services, Radiological Health, and Licensure and Certification
    • Complete its community health assessment and community health improvement plan
    • Apply for accreditation

2012 Funding (Year 3)

  • Amount of funding: $399,709
  • Description of Project
    Awardee will:
    • Develop and complete its community health assessment and community health improvement plan
    • Resolve all documentation gaps identified for accreditation by the Public Health Accreditation Board
    • Automate restaurant permit application and bedding and upholstery permitting processes
    • Continue to review and develop ongoing performance improvement projects

2011 Funding (Year 2)

  • Amount of funding: $399,859
  • Description of Project
    Awardee will:
    • Establish a public health accreditation team
    • Expedite the development of a timeline and roadmap
    • Organize agency workforce documentation
    • Develop a work-plan to develop a state health assessment and state improvement plan
    • Implement an agency-wide performance improvement project to automate paper-based processes and forms
    • Implement an ongoing best practice collaborative with cross-jurisdictional partners to identify and implement evidence-based policies

2010 Funding (Year 1)

Component I

  • Amount of funding: $300,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
