2013 Funding (Year 4)

  • Amount of funding: $573,986
  • Description of Project
    Awardee will:
    • Expand the performance dashboard that publically presents population objectives, performance measures, and trends over time
    • Facilitate the Department’s Public Health Stat process, a management tool that enables data-driven decision making through cross-divisional content-based meetings where decision makers come together to do program planning and resource allocation around high priority department-wide goals
    • Facilitate internal quality improvement activities that include providing training to quality improvement champions, identifying projects and priorities, and coaching staff through the improvement process
    • Enhance the use of data and promising or evidence-based practices to improve health outcomes through improved infrastructure to provide epidemiological data to staff and stakeholders and by funding community coalitions to implement evidence based practices
    • Implement and guide the Department’s integrated prevention team efforts by establishing model benchmarks, facilitating efforts at central office and the district offices, and providing technical assistance
    • Prepare for Public Health Accreditation Board site visit and follow-up activities

2012 Funding (Year 3)

  • Amount of funding: $607,600
  • Description of Project
    Awardee will:

2011 Funding (Year 2)

  • Amount of funding: $607,600
  • Description of Project
    Awardee will:
    • Prepare for accreditation readiness, enhance performance management and quality improvement activities, and expand the implementation of best and promising practices in prevention and performance management
    • Enhance and expande year one activities (i.e., building a comprehensive performance management and quality improvement system, implementing the use of integrated prevention teams, working with community coalitions to build capacity around healthy retailers and built environment promising practices, and undertaking work to prepare for national accreditation)
    • Offer CDC the opportunity to showcase best and promising practice implementation in a small rural state

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC

Component II

  • Amount of funding: $1,000,000
  • Description of Project:
    Awardee will enhance public health improvement efforts beyond Component I through two or more categories including: performance management, policy and workforce development, public health system development/redevelopment and best practice implementation.
