2012 Funding (Year 3)

• Amount of funding: $250,000
• Description of Project
  Awardee will:
  • Develop a community health assessment
  • Conduct an organization self-assessment against the Public Health Accreditation Board (PHAB) standards
  • Address deficiency areas in preparation for PHAB accreditation
  • Develop quality improvement initiatives, including improving HIV testing sharps disposal, improving customer satisfaction with Aging Services, and improving personnel hiring system
  • Develop an Office of Performance Improvement
  • Develop an internal web-based comprehensive health data management information system
  • Implement the activities in its infrastructure plan

2011 Funding (Year 2)

• Amount of funding: $250,000
• Description of Project
  Awardee will:
  • Continue to build and strengthen its public health infrastructure by implementing key components of the Navajo Nation Strategic Plan
  • Address identified deficiency areas in preparation for Public Health Accreditation Board accreditation
  • Develop a Navajo Nation-wide community health assessment
  • Develop a quality improvement initiative addressing the effectiveness of key services the Navajo Division of Health provides
  • Develop cross-jurisdictional partnerships with at least one local health department in each of the three states (i.e., New Mexico, Arizona, and Utah) where Navajo residents are located
  • Identify and implement traditional healing best practices in behavioral health related to alcohol, HIV, or diabetes

2010 Funding (Year 1)

  • Focus on building, institutionalizing, and implementing performance management capacity within health departments
  • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
