2013 Funding (Year 4)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Create a department website to disseminate information about its products, services, and relevant health or weather advisories
    • Develop tribal and community partnerships
    • Work toward developing a tribal health improvement plan
    • Build capacity with tribal elected officials (the governing entity)

2012 Funding (Year 3)

  • Amount of funding: $247,032
  • Description of Project
    Awardee will:
    • Complete the community-wide public health needs assessment, including promoting participation from the community
    • Contract with Great Lakes Inter-Tribal Council to analyze and evaluate needs assessment data
    • Hire a full-time Public Health Compliance Officer to implement Public Health Accreditation Board standards and measures to advance public health accreditation efforts
    • Conduct a community-wide marketing campaign to communicate public health initiatives and enhanced efficiencies

2011 Funding (Year 2)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Contract with outside software management agency and with a well-respected Native American public health institution for epidemiology training
    • Advance accreditation process to improve overall care of its patients
    • Access a software system to chart patients' data and enable an on-going dialogue between health professionals regarding patient care
    • Contract with Lac du Flambeau, a sister tribe, to learn methodologies for analyzing data and converting it to an epidemiology study
    • Research and apply for alternative funding sources to sustain public health programs in the future

2010 Funding (Year 1)

Component I

  • Amount of funding: $99,866
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
