2013 Funding (Year 4)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Complete a community health assessment of obesity status in maternal and child health
    • Analyze current primary data on obesity and its statistical relationship with diabetes and healthy living
    • Increase effectiveness in addressing obesity
    • Conduct data analysis on obesity, diabetes, and healthy living among participants of the Women, Infants, and Children (WIC) Program and select community organizations
    • Continue performance management activities in the agency and allied organizations

2012 Funding (Year 3)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Collect primary data to complete a health assessment
    • Decrease wait time in hospital emergency rooms
    • Decrease the time it takes to complete health facilities inspection reports
    • Provide regulation and accreditation training to health administrators

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Update its webpage that contains outdated information and statistics and its outdated data warehouse (DW)
    • Evaluate modifications to the Vital Statistics System
    • Update and improve the DW with a new architecture infrastructure using Microsoft SharePoint 2010 for publishing and presenting health information on the Internet
    • Maintain an operational infrastructure that supports the performance of public health functions
    • Use the DW and the Web-based Data Query System as tools to support their mission and to enhance their workforce's ability by providing the needed infrastructure for data analysis and communication
    • Contract with consultants for the DW project and Microsoft consultants to work with and train Puerto Rico Department of Health staff in every phase of the project
    • Complete a state health assessment of Puerto Rico
    • Develop an interactive training module for physicians, namely to address completion of the Cause of Death section on death certificates

2010 Funding (Year 1)

Component I

  • Amount of funding: $200,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
