2013 Funding (Year 4)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Complete its community health assessment
    • Complete an organizational self-assessment and work through a Quality Improvement Council to carry out improvement activities and address identified gaps
    • Conduct assessments of the hiring and billing processes to address opportunities for improvement in these areas
    • Plan for the implementation and evaluation of a pilot wellness program identified through the Evidence-Based Practices Advisory Group that will improve the coordination between the Tobacco and Diabetes Programs

2012 Funding (Year 3)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Develop a strategic plan and establish a Quality Improvement Council
    • Complete an organizational self-assessment against Public Health Accreditation Board standards and provide instructor-led training to identify deficiencies
    • Establish an Evidence-Based Practices (EBP) Advisory Group to provide guidance and actionable recommendations to senior leadership and the Performance Improvement Manager
    • Use a data-driven approach to identify two public health programs for EBP pilot implementation
    • Analyze and disseminate Year Two infrastructure assessment information
    • Increase the number of staff dedicated to performance management efforts

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Build the needed infrastructure to ensure continuous improvement in public health service delivery
    • Use routine and rigorous monitoring across the public health system to enhance capacity to monitor how well the health needs of population are being met and to increase efficiencies of program operations
    • Establish and staff a Public Health Performance Management Office
    • Complete and implement a five-year performance management plan to meet national public health performance standards
    • Operationalize a Performance Management System
    • Train 90% of staff in performance improvement protocols

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC

Component II

Component II funding was applied through Pacific Island Health Officers Association (PIHOA).
