2013 Funding (Year 4)

  • Amount of funding: $723,968
  • Description of Project
    Awardee will:
    • Continue to support the building of public health infrastructure and performance management
    • Refine the community health assessment process to a three-year cycle
    • Expand community health improvement plans for each county to involve local government, chamber of commerce, boards of education, and providers
    • Integrate Lean management principles facilitation into the Department’s current Health Systems Council process that addresses ineffective processes and inefficiencies as they arise
    • Create the requirements and navigation tool to promote performance management document sharing and best practice sharing using the Department’s Public Health Information Exchange
    • Complete the death registration electronic process and begin system testing and user acceptance testing for birth and the remaining modules to advance the electronic vital records information management system
    • Complete statewide training of all Department staff on customer focused government (customer service training)

2012 Funding (Year 3)

  • Amount of funding: $757,600
  • Description of Project
    Awardee will:
    • Complete the prerequisite documents to prepare for public health accreditation, including a needs assessment, strategic plan, and performance improvement plan
    • Build upon county health assessment activities completed in Year 2
    • Develop a workforce training plan based on results from a public health competency survey
    • Continue replacing the existing, largely paper-based vital records system with the new real-time electronic registration system, including streamlining its death certificate processing

2011 Funding (Year 2)

  • Amount of funding: $757,600
  • Description of Project
    Awardee will:
    • Complete a needs assessment, strategic plan, performance improvement plan, and inventory of primary and secondary health data systems
    • Develop tools to identify needs and evaluate health risks to assist with strategic and performance improvement planning at the state and local level
    • Train management staff on the fundamentals of performance management
    • Work with collaborative partners, knowledge experts, and others to provide performance management training to local health department staff and other frontline staff
    • Improve the operational infrastructure of vital records and county health departments
    • Deploy the Electronic Death Registration System and by the Electronic Birth Registration System

2010 Funding (Year 1)

Component I

  • Amount of funding: $296,995
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC

Component II

  • Amount of funding: $1,000,000
  • Description of Project:
    Awardee will enhance public health improvement efforts beyond Component I through two or more categories including: performance management, policy and workforce development, public health system development/redevelopment and best practice implementation.
