2013 Funding (Year 4)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Submit application to the Public Health Accreditation Board
    • Implement the workforce development plan
    • Continue quality improvement (QI) efforts department-wide
    • Establish the roles, process, and timing for annual review of critical documents for accreditation purposes, such as the workforce development, QI, health improvement, and strategic plans

2012 Funding (Year 3)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Complete the three Public Health Accreditation Board (PHAB) accreditation prerequisites
    • Complete the first wave of quality improvement (QI) projects and complete the QI plan
    • Complete the first round of documentation for the 12 accreditation domains
    • Advance its workforce development plan and learning management system
    • Submit accreditation application to PHAB

2011 Funding (Year 2)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Develop a public health road show, the "2012 RHODE Show," to build the Rhode Island Department of Health's (RIDOH) capacity for systematic public health planning
    • Create a routine and transparent process for measuring performance that incorporates the voice and input of community partners, other state agencies, nearby state health departments, and the general public
    • Establish statewide public health performance goals based on Health People 2020 and the five-tier health impact period
    • Understand the efforts of other agencies and community organizations that may be working on similar goals to provide RIDOH, as well as the state's governor and legislature, with a picture of the resources available to address critical public health needs
    • Develop long-standing collaborations with other organizations
    • Measure progress towards meeting goals
    • Develop a sustainable performance measurement/quality improvement system for data collection, exchange, and dissemination
    • Establish a small management team comprised of a cross-section of representatives from all areas and levels of the health department, who will be responsible for coordinating the grant activities

2010 Funding (Year 1)

Component I

  • Amount of funding: $99,738
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
