2013 Funding (Year 4)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Maintain good standing of state public health department accreditation and improve the efficiency, effectiveness, quality, and performance of the agency and its services
    • Assist county health departments in their accreditation readiness activities and continue accreditation coordinator network
    • Share best practices and strategies from the field
    • Conduct quality and performance improvement initiatives and reporting, including increasing reach and preventive behaviors around pre-term births and infant mortality and improving compliance with inspection frequency mandates

2012 Funding (Year 3)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Apply for public health accreditation
    • Assist county health departments in completing accreditation readiness activities
    • Convene the county accreditation coordinator network, a collaborative of accreditation coordinators from county health departments
    • Conduct quality/performance improvement initiatives and reporting

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Increase the capacity to routinely evaluate and improve system-wide effectiveness through the Step UP performance management system and related applications
    • Increase alignment of state services and county health departments with the state health improvement plan and strategic map priority goals
    • Increase collaboration and implementation of promising and best practices
    • Improve workforce performance management, strategic planning, accreditation readiness, and quality improvement knowledge, capacity, and skills

2010 Funding (Year 1)

Component I

  • Amount of funding: $200,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
