2012 Funding (Year 3)
- Amount of funding: $950,791
- Description of Project
Awardee will:- Launch a statewide online local health performance management system
- Complete a Public Health Accreditation Board gap assessment and strategic plan
- Continue healthcare-associated infection surveillance in ambulatory surgical centers and expand the surveillance and educational program to hospitals
- Launch the City of Trenton’s Health Information Exchange System and link it to the state and community partners’ data systems
- Provide technical assistance to county health departments to incorporate evidence-based policies and environmental changes into community health improvement plans for obesity prevention and reduction
2011 Funding (Year 2)
- Amount of funding: $950,791
- Description of Project
Awardee will:- Implement relevant and essential activities to accelerate their readiness to apply for and achieve national accreditation through the Public Health Accreditation Board (PHAB) for their state and local health departments
- Engage in cross-jurisdictional partnerships with one or more local health departments with a proven record of performance improvement adoption or implementation of public health policies to implement Trenton Health Team's Health Information Exchange, which aims to 1) enhance prevention of communicable diseases through immunization and by building upon the Healthcare Information Exchange project to ensure a continuum of care to City of Trenton residents in preventive, primary, acute, rehabilitation, long- term care, and supportive and ancillary services, regardless of insurance status, and 2) ensure a secure transmission of data between health care providers
- Create an Ambulatory Surgery Center (ASC) Healthcare-Associated Infection Surveillance Program within the Department of Health and Senior Services' (DHSS) Office of Health Care Quality Assessment and reduce surgical site infection rates within these facilities, which will complement DHSS' patient safety quality indicator surveillance in hospitals, expand infection control surveillance to ASCs, and ultimately, assist DHSS in decreasing surgical site infection rates in ASC statewide

- Description of Project:
Awardee will:
- Focus on building, institutionalizing, and implementing performance management capacity within health departments
- Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
Component II
- Amount of funding: $1,260,600
- Description of Project:
Awardee will enhance public health improvement efforts beyond Component I through two or more categories including: performance management, policy and workforce development, public health system development/redevelopment and best practice implementation.
