2013 Funding (Year 4)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Implement the state health improvement plan
    • Revise, implement, monitor, and evaluate a an organizational strategic plan as prerequisite to public health accreditation
    • Implement its performance management application within the construct of HealthWISDOM, a web-based interactive system for direction and outcome measures
    • Provide training in performance management practices and quality improvement methods and tools to its management and staff
    • Facilitate quality improvement activities and the institutionalization of a quality culture throughout the Division

2012 Funding (Year 3)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Prepare to apply for public health accreditation by completing prerequisite plans, readiness self-assessment, and the application roadmap and timeline
    • Provide training and technical assistance to management and staff to measure, monitor, and report on progress toward meeting performance management objectives
    • Provide training and technical assistance to management and staff on quality improvement techniques and tools
    • Develop a performance management information technology (IT) system
    • Assist management and staff with the use of the performance management IT system

2011 Funding (Year 2)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Inform staff about accreditation requirements and develop details, steps, and timelines to prepare for accreditation
    • Train Division of Public Health Services (DPHS) staff in performance improvement methodology and tools
    • Develop and utilize a web-based performance management system that allows ongoing systematic monitoring, evaluation, and reporting of DPHS performance
    • Identify and implement a greater number of promising and best practices
    • Implement best practice protocol in WIC clinics throughout the state to reduce the number of pregnant women smoking

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
