2013 Funding (Year 4)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Achieve national public health accreditation
    • Implement a state health improvement plan, which includes cross-cutting activities that focus on coordination with the implementation of the Affordable Care Act and better integration of the public health and healthcare sector, influencing communities, schools, work sites, and other settings to create healthier environments
    • Implement its strategic plan
    • Assess and further develop state, tribal, and local public health workforce
    • Continue to build a system for performance management and continuous quality improvement

2012 Funding (Year 3)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Implement essential activities to prepare for national public health accreditation
    • Institutionalize continuous quality improvement throughout the agency
    • Apply for national public health accreditation through the Public Health Accreditation Board
    • Support tribal and local public health departments to increase the number that are preparing to apply for national public health accreditation
    • Work with tribal and local health departments and/or regional public health collaboratives to facilitate accreditation technical assistance and training for their agencies
    • Increase the number of initiatives to implement evidence-based policies and procedures
    • Align the work of the Public Health and Safety Division with the state health improvement plan
    • Implement a performance improvement initiative to prioritizes activities, practices, and programs for maximum public health impact

2011 Funding (Year 2)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Prepare a state public health assessment, a state public health improvement plan, and a strategic plan for the Public Health and Safety Division
    • Develop a six-month quality improvement training program, led by Purdue University, for state and local health agencies
    • Build an integrated management system that ties together all program work and tracks performance through regular reviews and a performance management system
    • Assess public health service contracts to evaluate the number of evidence-based interventions used in contracts for the purposes of increasing the number of state and local programs using evidence-based interventions
    • Continue to prepare the Public Health and Safety Division and local health departments for accreditation
    • Increase the number of initiatives to implement evidence-based policies, regulations, and laws among local health departments by providing training and technical assistance to state and local staff on evidence-based policies and laws, such as those recommended by the Community Guide

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
