• Description of Project
  Awardee will:
  • Conduct a statewide community health assessment
  • Conduct a Public Health Accreditation Board self-assessment to determine opportunities for improvement and to prepare for accreditation
  • Continue to implement agency-wide performance improvement using Lean methodology with particular focus on the standardization and streamlining of agency fiscal processes
  • Collaborate with state and local partners to improve stewardship of resources and health outcomes

2011 Funding (Year 2)

• Description of Project
  Awardee will:
  • Continue building upon initial target areas for improving the public health system and infrastructure that were identified in Colorado's Public Health Improvement Plan: From Act to Action
  • Support future accreditation by the Public Health Accreditation Board by meeting critical standards

2010 Funding (Year 1)

Component I

  Awardee will:
  
  • Focus on building, institutionalizing, and implementing performance management capacity within health departments
  • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
