2013 Funding (Year 4)

  • Amount of funding: $400,000
  • Description of Project
    Awardee will:
    • Create a statewide public health improvement plan guided by Public Health Accreditation Board (PHAB) standards and informed by the statewide assessment
    • Continue organizational assessment and collection of documentation to meet PHAB requirements and prepare for accreditation application
    • Develop a performance management system that aligns with its Office of State Budget and Planning planned system
    • Improve processes and policies regarding the sharing of health and environmental data with internal and external partners
    • Improve process for inspecting assisted living facilities, given the increasing number of residences with aging population

2012 Funding (Year 3)

  • Amount of funding: $400,000
  • Description of Project
    Awardee will:
    • Conduct a statewide community health assessment
    • Conduct a Public Health Accreditation Board self-assessment to determine opportunities for improvement and to prepare for accreditation
    • Continue to implement agency-wide performance improvement using Lean methodology with particular focus on the standardization and streamlining of agency fiscal processes
    • Collaborate with state and local partners to improve stewardship of resources and health outcomes

2011 Funding (Year 2)

  • Amount of funding: $400,000
  • Description of Project
    Awardee will:
    • Continue building upon initial target areas for improving the public health system and infrastructure that were identified in Colorado's Public Health Improvement Plan: From Act to Action
    • Support future accreditation by the Public Health Accreditation Board by meeting critical standards

2010 Funding (Year 1)

Component I

  • Amount of funding: $300,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
