2013 Funding (Year 4)

  • Amount of funding: $1,037,858
  • Description of Project
    Awardee will:
    • Apply to the Public Health Accreditation Board and achieve accreditation
    • Finalize a state health assessment and develop a state health improvement plan
    • Implement the department’s strategic plan
    • Provide training on quality improvement and return on investment, as well as training and resources to assist local health departments in achieving accreditation
    • Develop a department-wide performance management system
    • Engage two additional teams in a quality improvement project

2012 Funding (Year 3)

  • Amount of funding: $1,093,662
  • Description of Project
    Awardee will:
    • Use its new Office of Quality Performance and Accreditation to implement quality improvement initiatives and provide training and technical assistance to its staff
    • Perform gap analysis for public health accreditation readiness
    • Create a California Performance Improvement Management Network with local health departments

2011 Funding (Year 2)

  • Amount of funding: $1,093,662
  • Description of Project
    Awardee will:
    • Provide assistance to local health departments (LHDs) to assess accreditation readiness and to develop and implement performance improvement measures
    • Work with Los Angeles County and San Diego County to share and review best practices and coordinate a one-day conference to meet with LHDs to discuss performance improvement indicators and best practices
    • Establish a public health infrastructure team to assess and identify development needs of California's public health information system infrastructure
    • Complete assessments and develop a timeline and roadmap to the department's application for accreditation
    • Increase capacity to receive lab, immunization, and syndromic surveillance data sent by hospital and provider electronic health record systems
    • Develop a centralized metadata resources repository to provide an infrastructure for data collection and analysis, program management, and communication
    • Administer performance management and quality improvement trainings and health informatics training to California Department of Public Health and LHD staff members
    • Develop a set of new performance management standards and measures
    • Explore performance management tracking systems with partners in San Diego and Los Angeles counties, and adapt a system tailored to the department's organizational structure and needs
    • Conduct cost-benefit analysis of public health interventions to improve business processes

2010 Funding (Year 1)

Component I

  • Amount of funding: $400,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC

Component II

  • Amount of funding: $1,660,128
  • Description of Project:
    Awardee will enhance public health improvement efforts beyond Component I through two or more categories including: performance management, policy and workforce development, public health system development/redevelopment and best practice implementation.
