  Awardee will:
  • Develop and implement a community health improvement plan with stakeholders and partners
  • Conduct two quality improvement initiatives to improve effectiveness and efficiency in Women Infant and Children (WIC) Clinics and in its partnerships with schools
  • Improve cross-jurisdictional work among departmental programs and with community partners
  • Complete a public health accreditation readiness assessment

  • Facilitate a strategic planning process that improves program outcomes
