2013 Funding (Year 4)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Create a departmental strategic plan
    • Focus on building, institutionalizing, and implementing performance management capacity within the health department
    • Build and develop a departmental accreditation team
    • Increase workforce capacity, formalizing a workforce development plan and prioritizing training needs
    • Conduct quality improvement initiatives to improve efficiency in contracting with schools and improve effectiveness by increasing the number of evidence-based approaches used in the department

2012 Funding (Year 3)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Develop and implement a community health improvement plan with stakeholders and partners
    • Conduct two quality improvement initiatives to improve effectiveness and efficiency in Women Infant and Children (WIC) Clinics and in its partnerships with schools
    • Improve cross-jurisdictional work among departmental programs and with community partners
    • Complete a public health accreditation readiness assessment

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Complete a community health assessment
    • Create a community health improvement plan
    • Facilitate a strategic planning process that improves program outcomes
    • Increase the use of evidence-based policies and practices

2010 Funding (Year 1)

Component I

  • Amount of funding: $199,434
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
