2013 Funding (Year 4)

  • Amount of funding: $400,000
  • Description of Project
    Awardee will:
    • Finalize and communicate the results of a state-level health assessment
    • Work with state partners to prioritize health issues and to create and implement a state-level health improvement plan
    • Implement an agency-wide performance management system
    • Strengthen the culture of continuous quality improvement (CQI) at the agency and continue to build the CQI training program to include a belt system with beginner to advanced levels
    • Revise the strategic plan annually to ensure the goals continue to align with program, partner, and agency goals
    • Complete accreditation readiness activities, finalize Public Health Accreditation Board domain documentation, and apply for accreditation

2012 Funding (Year 3)

  • Amount of funding: $400,000
  • Description of Project
    Awardee will:
    • Collaborate with local health departments to conduct community health assessments
    • Develop an external-facing information technology infrastructure to provide counties with primary data collection support
    • Establish itself as an academic health department with a cross-jurisdictional relationship with the University of Arizona
    • Develop an internal SharePoint site to house documentation required by the Public Health Accreditation Board and other supportive documentation for the state health assessment, State Health Improvement Plan, and advisory team meetings
    • Develop a performance management system

2011 Funding (Year 2)

  • Amount of funding: $400,000
  • Description of Project
    Awardee will:
    • Increase efficiencies of program operations, workforce development, and use of evidenced-based policies and practices
    • Collaborate with system partners to improve state-wide health outcomes while preparing the agency for accreditation
    • Expedite public health accreditation readiness activities, including the implementation of the Arizona Department of Health Services strategic plan, the development and monitoring of performance measures using national standards, the planning and implementation of a state health improvement Plan, and the development of an agency-wide quality improvement plan

2010 Funding (Year 1)

Component I

  • Amount of funding: $289,586
  • Description of Project
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
