Case Notification Message Mapping Guides

This page provides case notification message mapping guides and other artifacts used for messaging HL7 v2.5 data to CDC.

To download a Message Mapping Guide (MMG) and other messaging artifacts, click on the MMG you would like to implement.  Please reference the 2010 Event Codes and Reporting Mechanisms document, which list the event codes for the nationally notifiable diseases and other conditions of public health importance for January 2010 with a notation for the reporting mechanism.
