Due to the emergence of the novel influenza A (H1N1) virus over recent months and its potential to become a pandemic, the CDC Countermeasure and Response Administration (CRA) team has refocused efforts to tracking H1N1 vaccine doses administered during the fall of 2009. All Public Health Emergency Preparedness (PHEP) grantees (collectively referred to as Project Areas and include the 50 states, 8 island territories and 4 major metropolitan areas) are expected to report vaccine doses administered to the CDC according to age groups determined by the Advisory Committee on Immunization Practices (ACIP) using one of three options via the CRA system.

Current efforts towards vaccine manufacture and production are ongoing, and H1N1 vaccine is estimated to be ready for distribution by mid to late fall of 2009. Timely and complete reporting of H1N1 vaccine doses administered is critical for both the federal government and Project Areas to monitor the utilization of vaccine while it is in limited supply. To support this effort, the CDC CRA team is currently enhancing CRA’s capabilities and preparing for this extensive event.

Over the previous two years, Project Areas have prepared for a large scale vaccine doses administered event by participating in the 2007 pilot test and DAX2008 exercise. With the emergence of pandemic potential novel H1N1 Influenza A, the planned DAX2009 has been put on hold indefinitely with efforts refocused to reporting H1N1 vaccine doses administered. The H1N1 vaccine is expected to be limited in supply during the initial months of availability and will need to be monitored to ensure the US population is vaccinated according to ACIP determined age groups. CRA will be used to collect, aggregate, report and analyze H1N1 vaccine doses administered data. In addition, CDC will monitor timeliness and completeness of reporting data to maintain quality control.

Project Area Requirements

H1N1 vaccine doses administered is several magnitudes larger and more complex than CRA exercises to date. However, all of the methods and approaches used for the exercises are applicable and help serve Project Area preparedness to support the H1N1 event. Project Areas will be required to expand efforts to include all mass immunizations state-wide. Requirements for the H1N1 response include:

  • Prepare, collect and report all mass vaccination clinic aggregate or detail data, depending on option chosen
  • Data collection and reporting to include all local jurisdictions and identified clinic sites
  • Aggregate and report data weekly to CDC by Tuesday for the previous reporting week (Sunday through Saturday; MMWR week)
  • Report the minimum data set beginning from when the vaccine is first received until CDC signals to stop

Age Group Tracking

  • Vaccinees must be assigned to ACIP determined Age Groups.
  • Project Areas must have a method in place to screen vaccinees into these age groups.

H1N1 Vaccine Doses Administered Event Planning at Project Area Level

To help ensure an effective mass vaccine event, Project Areas are advised to begin planning efforts immediately. Resources and needs for the administration and tracking of the H1N1 vaccine, as well as evaluation and improvements in communication flow should be identified as necessary within each Project Area. Planning efforts should include, but are not limited to, the following: 

Select a Data Reporting Option

  • Designate option choice as one of the following:
    • Data exchange (Option 1)
    • Direct web entry, aggregate (Option 2)
    • Direct web entry, individual (Option 3)
  • Confirm option choice with CDC CRA point of contact

Resources

  • Coordinate, identify, and recruit all necessary staffing and volunteer resources
  • Coordinate technical resources (e.g. digital certificates, IT, etc.)
  • Prepare and communicate training materials
  • Coordinate with preparedness officer and local health departments

Clinic Sites

  • Coordinate with local health departments to identify clinic sites, staff, volunteers and other necessary resources.
  • Plan for mass vaccination clinics as necessary; identify volunteers to support, administer vaccine and set up such clinics.
  • Ensure clinics have an internet connection system and have digital certificates.
  • Determine changes needed in order for clinics to be prepared for the real event.

Vaccine Administration Sites

  • Coordinate with preparedness and immunization programs to set up vaccine administration sites for the vaccine doses.
  • Many vaccine administration sites will be the clinics used for influenza vaccine; however; there may also be additional mass vaccination clinics.

Communication

  • Communicate and set expectations and guidance for both strategic and operational resources in order to report to CRA.
  • Establish call-back system for every clinic/operating site in order to follow up for timely information.

H1N1 Vaccine Doses Administered Event Planning at CDC Level

CDC will be preparing for the H1N1 vaccine doses administered event in parallel with Project Areas. Included in these efforts are:
  • Modifications in CRA application
  • Reconfirming CRA option choice with Project Areas
  • Completing necessary changes in data exchange specifications
  • Developing and communicating training materials and guidance
  • Investigating alternatives to digital certificates
  • Developing process and layout for reporting data
