Blue Square Bullet Nashville General Hospital at Meharry (NGHM)
“Strengthening Existing National Organizations Serving Racial and Ethnic Populations Capacity Development Programs:  Strategies to Advance Program Implementation, Coordination, Management, and Evaluation Efforts.”
 

The NGHM Center for Lifestyle and Health Management is a cooperative agreement sponsored by the CDC / OCPHP / OMHD and the NIH / NCMHD.

 

The center provides primary risk prevention and wellness services as well as secondary management services to racial and ethnic groups suffering from diabetes or CVD or both.  The program is designed primarily for those persons with mild hypertension, hypercholesterolemia, early diabetes, mild to moderate obesity; or those with family histories of CVD or related  complications.

 

This cooperative agreement supports the CDC goals:

     (1) Healthy People in Every Stage of Life, and
     (2) Healthy People in Healthy Places.

The purpose of the program is to:
  The CLHM seeks to provide an educational risk reduction program for individuals vulnerable to complications of diabetes and cardiovascular diseases.  In collaboration with primary care physicians, encourage clients to make lifestyle changes through a series of educational modules targeting:  exercise, nutrition, weight control, smoking cessation and stress management.
Specifically, the program is intended to:
  Blue Sphere Bullet Control adverse health outcomes among the residents of North Nashville who are diagnosed with any of the associated risk factors for diabetes or CVD or both.
  Blue Sphere Bullet Improve the quality of life for residents of North Nashville who have pre-diabetes or cardiovascular risks, as well as for those patients now suffering from diabetes or CVD.
Blue Sphere Bullet Reduce the secondary and territory health consequences and effects such as dialysis, amputations, and obesity in patients who would otherwise continue to have unmanaged hypertension or diabetes or both.
Blue Sphere Bullet Achieve both cost savings within, and reduced systemic burdens on, the local health care safety net facilities from patients and residents suffering from diabetes or CVD or the associated pre-disease risk factors.
Blue Sphere Bullet Educate NGHM and area safety net physicians and other providers, as well as patients and their families about the risk factors and triggers associated with diabetes, CVD, stroke, and the medical and lifestyle alternatives which will help prevent and treat these conditions.
Blue Sphere Bullet Empower patients and their families to manage their own health and wellness independently, to know when and where to seek further help, and to reduce their need for medical care.

The purpose of the Grant is to:
  The purpose of this grant is to develop a Minority Health, Education, Research and Outreach (MiHERO) Center. 

This initiative will build upon ongoing research programs, clinical care, and educational efforts (including community engagement programs) aimed to understand the underlying causes of disparities and devising innovative strategies to ultimately eliminate them.

The MiHERO Center takes an interdisciplinary approach to address the multiple factors influencing infant mortality and cancer at different levels in order to take a multilevel approach to outreach activities.

Health disparities are conceptualized as the result of complex interactions among biological, psychosocial, and environmental risk and protective factors that accumulate across a life course and influence not only the outcome of an individual, but also those of subsequent generations. 

The goals for the first year of funding are: 

(1) to provide a national leadership model in public health professional education; and

(2) serve as a central resource for the minority communities in North Texas to disseminate health information and promote participation in health programs.
