Changes in Data Collection: End of Data Collection
for Current NPHPSP Tools
To prepare for the release of the Version 2 instruments,
data collection for the NPHPSP Local Public Health
System and State Public Health System Performance
Assessments will stop on May 1, 2007. State
and local jurisdictions using these instruments
should plan to have all data entered into the online
data reporting system before this date.
To obtain a User ID, please use the online User
ID request form available at
http://www.nphpsp-results.org.

or call 202-218-4411. Please be sure to indicate
your health department name and state, as well as
a contact name and contact information.
As previously communicated, data collection for
the Governance instrument ended on December 31,
2006 (in accordance with OMB approval of these data
collection activities). Any boards of health that
have used this instrument and have not submitted
their responses should contact CDC NPHPSP staff
at phpsp@cdc.gov
or contact Jennifer O’Brien at NALBOH at
Jennifer@nalboh.org.
All users (state public health systems, local
public health systems, and boards of health) that
have already submitted data will still be able to
use their User ID to access their previously-generated
