Import Permit Applications
Please note that the new Office of Management and Budget (OMB) approved Import Permit Forms (expiration date 1/31/2014) are now available and supersede the previous forms. Expired forms cannot be accepted for processing.

***Please email or fax only one copy of your completed, signed application to the Etiologic Agent Import Permit Program (EAIPP) at the Centers for Disease Control and Prevention (CDC). You do not need to mail a hardcopy of your application to the EAIPP if you have already provided it via email or fax transmission.***
