The NPHPSP assessment instruments were originally released in 2002 and utilized by more than 30 states through 2007. The materials below are related to the original release (Version 1) of the NPHPSP instruments. Current versions of the instruments and related materials (Version 2) were released in August 2007. After public health systems and governing bodies have completed the assessment instrument (see the User Guide for tips on how to undertake the process), responses can be submitted to a limited-access website for analysis and reporting.
