Abstract
The occupational health record should provide data for use in job placement, health maintenance, workmen's compensation cases and treatment and rehabilitation of occupational conditions. The record should also yield data for use in epidemiological studies, and establishing health standards, and it should contain data that would help management, both company and clinic, with program evaluation and improvement. Descriptions of medical facts, findings, diagnoses, and procedures must be uniform and should lend themselves to reliable statistical comparison and analysis. A good data base contains a patient's profile, including a past medical history, review of symptoms, family history, occupational history, physical examination findings, lab reports, x-ray reports, and other test results. Records should be updated yearly, the purpose and use of a summary sheet is also described. (Contract No. 210-77-0135)
