Hemovigilance Incident Reporting

Summary

The objectives of this training are to describe the key terms used in incident reporting, why error reporting is important, why employees don’t always report mistakes, using process codes to categorize errors, using the Tables of Instructions, completing your Monthly Reporting Plan, completing and entering a Hemovigilance Incident form in NHSN, Incident result and root cause analysis definitions, real case examples, completing the Incident Summary data form, linking Incidents with Adverse Reactions, and, finally, creating custom fields and labels.

Audience: This training is designed for staff in Blood Transfusion Services departments who will be collecting and/or entering incident report data into the Hemovigilance Module of NHSN and for staff who will be analyzing Incident report data.
