5 Steps to Get Started
Step 1. Complete the Required Training
Complete the required trainings below.

Approximate time - 4 hours
Step 2. Review and Accept Rules of Behavior & Register Your Facility
After agreeing to the Rules of Behavior, you will be taken to the NHSN Registration page.
After you register your facility, NHSN will send you an email containing the website and password to apply for a digital certificate.
NOTE: Please make sure that your email system will not block emails from nhsn@cdc.gov and PHINTech@cdc.gov
Approximate time - 5 minutes
Step 3. Obtain Your Digital Certificate & Complete Forms
After you register your facility, NHSN will send you an email containing the website and password to apply for a digital certificate.
Approximate time for CDC to process the digital certificate request 3-4 days.
Download and complete the following:

- Review System Requirements
Approximate time to install digital certificate 30 minutes (if system is configured properly to accept it).
Step 4. Complete Enrollment Forms Online
With completed step 3 forms in hand, access SDN and select “NHSN Enrollment” then click “Enroll a Facility.” Enter the information from the forms into the data entry screens. You must complete this in one online session; you cannot save incomplete data.
CDC recommends that this step be completed on or after Nov. 1, 2010, when an updated user agreement and consent will be available.
Approximate time - 30 minutes
Step 5. Print, Sign and Return Signed Consent Form to CDC
Once you submit the enrollment documents online, you will receive an email containing a link to the Agreement to Participate and Consent document. Read and print this document and obtain the required signatures.
Although it is not mandatory, CDC recommends giving this Q&A document, along with the NHSN participation agreement, to the executive who will sign your facility’s paperwork. In addition, please find a general Q&A about the new NHSN agreement here.
Fax the signed document to CDC, attention NHSN Administrator at 404-929-0131, or mail original by surface mail to NHSN Administrator, MS A-24, Centers for Disease Control and Prevention, 1600 Clifton Road, NE, Atlanta, GA 30333. Once received, CDC will activate your facility and notify you that you may access the NHSN Reporting tool.
