Summary of Project
North Carolina’s Community Health Assessment (CHA), enhanced in 2002, established a successful assessment project at the local level. However, this model lacks the critical components necessary to make it a comprehensive and fully integrated CHA system. This funding would enable North Carolina to complete the CHA system by developing or improving three main components: 1) Link local CHA findings to state public health program and policy development, and expand the CHA website to provide links to all CHA documents; 2) Provide CHA training and skill development to the local public health workforce; 3) Enhance the State Center for Health Statistics Website by creating County Profile pages summarizing major health indicators, providing a link to the data that support the 2010 health objectives into GIS Health Atlas. The work of this proposal will build the capacity of the state, local health departments, and their collaborating partners to use the CHA process to mobilize community health improvement through program planning, research, policy development, and resource allocation.
