Summary of Project
The New Mexico Department of Health (NM DOH) requests funding to continue to develop the Community Health Assessment Program (CHAP), building on momentum gained from previous efforts funded by previous grants. In the spring of 2007, New Mexico decided to adopt Utah’s IBIS system for web-based data dissemination, and intends to make web-based data queries available for local health data and indicators. The recent development of 33 county and 5 tribal health councils, in lieu of local health departments, provides a basis for community based health programs. The previous grant funding provided the development of New Mexico’s Community Health Assessment Program (CHAP), which trains local health councils in comprehensive health assessment, in contrast to legacy “silo” funding focused solely on specific diseases or issues.

New Mexico is a diverse state with no single racial / ethnic group majority. Residents are 11% American Indian and 59% Hispanic or non-White in 2004, and the population has poor health and socio-economic indicators, ranking below most states for poverty, health insurance coverage, injury deaths, and alcohol and drug-related deaths. NM DOH suffered deep funding cuts in 2003-2004. The impact is also high due to the partnerships supporting this project, affecting tribal health agencies in other states, and by providing a model applicable to other US-Mexico border states. This grant would provide crucial funding for the continued development of CHAP, and for the tribal epidemiologist position, the liaison for tribal health partnerships.
