Summary of Project
New Hampshire proposes to extend its existing community health data reporting systems with innovative new modules that for the first time will allow New Hampshire’s public health partners in hospitals, community health agencies, local public health collaborators, and health departments, to access risk factor, chronic disease prevalence, and health assessment measures specific to the communities they serve. In 2004, New Hampshire Department of Health and Human Services (DHHS) began the development of the New Hampshire web-based reporting and query system (NH HealthWRQS), incorporating birth, death, cancer incidence, inpatient hospital discharges, and emergency department / urgent care visit data sets, to develop community health indicator reports. NH HealthWRQS will be completed and rolled out to local community users in April 2007. The New Hampshire Assessment Initiative application proposes to significantly augment the current NH HealthWRQS by: 1) developing a BRFSS web-based module that would allow for the calculation of custom weighted point prevalence estimates for risk factors, and confidence intervals, based on user-defined geographic areas; 2) developing a healthcare claims module that would estimate the prevalence of disease and provide measures of health care access; 3) defining a data set and web-based module of local public health program performance measurement data; and 4) forming a more comprehensive Community Health Profile that includes indicators from NH HealthWRQS, as well as indicators from the new modules. The development of these modules is expected not only improve the ability to monitor community health objectives in New Hampshire, but also improve the knowledge-base for developing web-based data dissemination systems for other states.
