Summary of Project
The Florida Department of Health (FL DOH) plans to sustain community health assessment practice in local county health departments. Florida’s health assessment program is well-established, due to the previous 5 years of the Comprehensive Assessment, Strategic Success initiative (COMPASS). Florida boasts a historical legacy of evidence-based or data-driven assessment. Electronic health data were made available in the 1990s through Florida’s Public Health Indicators Data System (PHIDS), which was replaced by the web-based Community Health Assessment Resource Tool Set (CHARTS) in 2004. The program has established strong internal and external partnerships, and has strengthened capacity for theory and practice by deploying various assessment models. However, despite this robust practice, currently more than 60% of county health departments rate their ability to sustain strategies as moderate to low. The proposal hopes to promote the practice of community health assessment in Florida to the level of model practice. Funding will staff one position and otherwise support and train county health departments and community partners.
