3A. Define and pursue a science-based approach to identify and eliminate health disparities related to HIV/AIDS, viral hepatitis, STDs, and TB and associated diseases and conditions.

I. Develop, implement and evaluate a surveillance and public health prevention research agenda for addressing health disparities and fostering health equity in HIV/AIDS, viral hepatitis, STDs, and TB through comprehensive and multi-level approaches including addressing the social determinants of health.

Indicator: Annual publication of integrated surveillance report that routinely address absolute and relative health disparities (by 2015)

FY 2011-2012 Accomplishments

  • Published NCHHSTP SDH white paper.
  • Published Public Health Reports special issue on data systems and SDH.
  • Published a review of current SDH variables collected in NCHHSTP surveillance systems.
  • Published paper on impact of SDH on HIV disease rates using correlation analysis of area-based summary information.
  • Sponsored 2011 Health Equity Symposium for CDC staff focusing on use of SDH data.
  • Initiated a work group to develop model SDH and health equity language for NCHHSTP communication products in collaboration with divisions.
  • Issued call for papers for Public Health Reports special issue on applying SDH in 2013.
  • Published FY11 SDH Activities Report cataloging NCHHSTP SDH-related activities.
  • Evaluated Geographic Information System (GIS) Supplement to the National HIV Surveillance Program in collaboration with CDC HIV Incidence and Case Surveillance Branch.
  • Presented information on HIV and STD clusters in geographical areas near HBCUs.
  • Drafted a proposal for a special issue of Women and Health on women in correctional facilities and health equity.
  • Conducted a focus group on health resources for adolescent girls.
  • Completed eighth year of NCHHSTP Third Thursday Lunch and Learn Health Equity Series.
  • Planned town hall meeting in collaboration with NCHHSTP HEWG to discuss applying SDH.

  • Fine tune evaluation process for strategic plan objectives to improve measuring and reporting annual progress.

II. Prioritize investments with explicit goals in programs and interventions to eliminate health disparities and promote health equity among all populations by working in collaboration with the NCHHSTP divisions and branches.

FY 2011-2012 Accomplishments

  • Worked with Divisions to plan, track and summarize investment activities pursuant to White House Orders for historically black colleges and universities (HBCUs), Hispanic serving institutions (HSIs), American Indian and Asian serving institutions.
  • Supported research and development of a curriculum for parents and caregivers to improve discussions regarding health sexuality with their children.

  • Work with Divisions to plan, track and summarize investment activities pursuant to White House Orders for HCBUs, HSIs, American Indian and Asian serving institutions.

III. Ensure that all funding opportunity announcements include a component that addresses reducing health disparities and promoting health equity.

IV. Promote, support, and evaluate evidence-based approaches on recommended hepatitis A and B and HPV vaccinations, and screening for HIV, viral hepatitis, and STDs for providers who serve predominately disproportionately affected populations in public and private healthcare settings.

FY 2011-2012 Accomplishments

  • Presented evidence-based evaluation of health care providers perspectives on barriers to prenatal syphilis testing and treatment, Shreveport, LA (2012 EIS Conference).

3B. Mobilize partners and stakeholders to promote health equity and social determinants of health as it relates to HIV, viral hepatitis, STD, and TB prevention.

I. Develop, implement and evaluate a communication science plan that articulates the vision for achieving health equity through comprehensive and multi-level approaches that are culturally appropriate and relevant.

FY 2011-2012 Accomplishments

  • Drafted communication science plan.
  • Enhanced SDH website.
  • Hired Health Communication Specialist.
  • Enhanced SDH website.
  • Developed model SDH and health equity language to be used in NCHHSTP products including FOAs.

FY 2013 Plans

  • Expand the number and type of key partners reached by communication strategies and identify missed opportunities to target important advocacy groups. Improve efforts to foster broad and consistent messaging across critical stakeholder groups.
  • Finalize and publish communication science plan.
  • Finalize and disseminate regular communication product (i.e., newsletter) with internal and external partners.
  • Develop a draft plan for a social determinants of adolescent health webpage.

II. Foster collaboration within CDC and among other federal and Department of Health and Human Services agencies to facilitate the conduct of public health research to eliminate health disparities and to integrate studies of social determinants of health.

Indicator: Comprehensive surveillance and research agendas for promoting health equity in focus diseases are written and vetted (by 2011)

FY 2011-2012 Accomplishments

  • Pubilshed and disseminated rapid ethnographic assessment results on contextual factors contributing to increased rates of syphilis among young African American women in Shreveport, LA.
  • NCHHSTP HEWG met monthly to update members on center and division activities and to communicate on activities.
  • Support OMHHE Migrant Health Stream Forum for FY 2012.
  • Corrections Work Group met every other month to plan activities for infectious disease prevention in correctional settings and share division updates.
  • Provided varied mentoring opportunities for racial/ethnic minority undergraduate and graduate students.
  • Supported CDC and HHS efforts to invest in support for minority scientists.

FY 2013 Plans

  • Continue support of OMHHE Migrant Stream Forum.

III. Continue to build community capacity and improve partnerships with communities affected by HIV/AIDS, viral hepatitis, STDs, and TB.

  • Published SDH FAQs, Glossary of Terms, and Training/Awareness slides.
  • Conducted a workshop on rapid ethnographic assessments as a tool for health equity at the National STD Prevention conference.
  • Supported the annual commemoration of the Presidential Apology for the U. S. Public Health Service Syphilis Study at Tuskegee Alabama.

IV. Create global partnerships to better characterize and address health disparities internationally and to protect the health of persons establishing residency in the United States.

FY 2011-2012 Accomplishments

  • Collaborated with Kenyan NGOs and Kenyan Ministry of Health on a rapid syphilis testing initiative.

FY 2013 Plans

  • Publish data evaluating rapid syphilis testing initiative to antenatal care services in Western Kenya.

3C. Identify which social determinants of health are important to address to reduce health disparities in HIV/AIDS, viral hepatitis, STDs, and TB and develop and advance appropriate plans for addressing these social determinants in NCHHSTP programmatic and scientific work.

I. Support expert consultations to assist NCHHSTP in prioritizing policy, programmatic, and research efforts based on the social determinants of health that have the greatest impact in reducing health disparities.

FY 2011-2012 Accomplishments

  • Held 2011 Health Equity symposium.
  • Published the Public Health and Homelessness Meeting Report and Resources.
  • Published a number of manuscripts in the 2011 special issue of Public Health Reports that identify important social determinants for reducing health disparities (e.g., Song, et al., 2011 and Beltran, et al., 2011).
  • Planned town hall meeting in to discuss applying SDH to NCHHSTP programs.

FY 2013 Plans

  • Identify social determinants of health that can be addressed consistent with the mission and core functions of public health. Identify resources and partnerships that can be leveraged to maximize opportunities to tackle SDH.

II. Develop strategic, multidisciplinary, and collaborative activities to facilitate novel cross-programmatic activities to reduce health disparities due to various social determinants of health.

  • Sponsored trainings for Health Impact Assessment and Rapid Ethnographic Assessment for NCHHSTP staff to use as a tool for acting on SDH.

FY 2013 Plans

  • Sponsor additional trainings for NCHHSTP staff on novel methods that adress health equity.

III. Develop or refine methodologies for obtaining, analyzing and presenting data on social determinants of health for NCHHSTP diseases using existing or novel supplemental surveillance systems.

V. Enhance partnerships with both traditional and non-traditional sources to strengthen the social determinants of health effort.

FY 2011-2012 Accomplishments

  • Consulted with researchers on NIH-funded research into the health and SDH-related experiences of Black men.
  • Collaborated with OSTLTS to support a National Network of Public Health Institutes feasibility study regarding the development of a Tribal Public Health Institute.

FY 2013 Plans

  • Determine feasibility of developing a Tribal Public Helath Institute.
