Community Health and Health Equity Program builds healthy communities and eliminates health disparities.
We provide national leadership in community health promotion and disease prevention through innovative programs; such as, Racial and Ethnic Approaches to Community Health (REACH), and State-Based Epidemiology for Public Health Programs (STEPPS). These cross-cutting programs help expand CDC’s ability to develop community-based health promotion strategies.

A three-year pilot project supporting teams that join the social movement to eliminate racial and ethnic health disparities. The project equips community teams with the knowledge and tools necessary to launch and sustain an effective local community action plan.
