Eligibility Requirements for Employer Participation in the National Healthy Worksite Program
Please read the following eligibility requirements for participation [PDF - 169KB] in the National Healthy Worksite Program. If you have any questions or need clarification about any of these requirements, please email NationalHealthyWork@cdc.gov. Use the subject line “Employer Certification.”
If your business MEETS the eligibility requirements, please click on the link to certify that you have read, understood, and meet the program eligibility requirements and provide your contact information between January 20th, 2012 and April 15th, 2012 to be contacted for a telephone interview in Spring 2012 to be considered for participation in the National Healthy Worksite Program.
If your business DOES NOT MEET the following eligibility requirements, please click on the link to provide your contact information to participate in worksite wellness training opportunities and to receive ongoing NHW program communications.
