CDC has searched to find available resources to help you plan, build, promote, and assess your obesity prevention and control program.  Below are some examples for developing your organization's tools. You can customize them to reflect your organization's needs and the needs of your employee population.

  • Business Case PowerPoint Template (PPT-734k) 
    A Business Case PowerPoint can help you present your program to senior management.  You can customize the sample PowerPoint above by providing information specific to your organization.
  • Committee Invitation (DOC-28k)
    Invite employees to participate in your organization's planning committee. You can customize the sample committee invitation letter above by providing information specific to your worksite program.
  • Committee Checklist (DOC-28k)
    A committee checklist can help you stay organized as you create your program committee.  You can customize the committee checklist above by adding names and information specific to your worksite program.
  • Employee Interest Survey*
    An employee interest survey can help you determine what your employees really want and need from your program. You can customize the sample survey above by providing information specific to your worksite program.
  • Health Risk Appraisal (HRA)* (PDF-637k)
    A health risk appraisal can help you learn more about your employees' physical activity and nutrition behaviors and develop a program that best meet the needs of your employees. Use this HRA as a template to customize a HRA to meet your employee needs.

  • Obesity Cost Calculator Worksheet (DOC-492k)
    This worksheet can help you gather the data you need to enter into the cost calculator application (provide link).  If you do not have data available, the Obesity Cost Calculator provides industry averages to assist you with completing the cost estimates.
