This manual is based on the collective experiences of many people working with the Centers for Disease Control and Prevention's (CDC) guidance document, Evaluating CDC-Funded Health Department HIV Prevention Programs (i.e., the Guidance). I want to express my appreciation to the following State Health Department staff and consultants who shared their insights with me during in-depth interviews: Corey Scott Bebarta and Tami Hoxworth, Colorado; Jim Welsh, Delaware; James Girvan, Lisa Kramer, Sarah Toevs and Anne Williamson, Idaho; Mark Miller, Los Angeles; Bob Woods, Maine; Maddy Shea, Maryland; Kevin Cranston, Massachusetts; Liisa Randall, Michigan; Gary Novotny, Minnesota; Marcia Sass, New Jersey; Angela Street-Underwood, Ohio; Viva Combs and Ali Mansaray, South Carolina; Khadi Ndao-Brunelay, Vermont; Marquietta Alston, Virginia; Mari Gasiorowicz, Wisconsin; and Randy Pope from the National Association of State and Territorial AIDS Directors (NASTAD). Numerous other individuals shared their thoughts about the Guidance during informal discussions and conference calls. Thanks are also due to the health departments that provided materials included in this document: Colorado, Maryland, New Jersey, Virginia, and Wisconsin.

Several people reviewed initial drafts of this document and provided invaluable feedback: Marquieta Alston, Virginia Department of Health; Melissa Beaupierre, Florida Department of Health; Hope Cassidy-Stewart and Maddy Shea, Maryland Department of Health and Mental Hygiene; David Cotton, ORC Macro; Maria Courogen, Nancy Hall, Frank Hayes, John Peppert, Leslie Pringle, and Amy Manchester Harris, Washington State Department of Health; Alan Friedlob, consultant; Mari Gasiorowicz, Wisconsin Division of Public Health; Ali Mansary, South Carolina Department of Health and Environmental Control; Claudia Montagne, consultant; Randy Pope, NASTAD; and Ann Willimason, Idaho Department of Health and Welfare.

I would also like to acknowledge Marlene Glassman, Carl Hill, and Romel Lacson, CDC, Division of HIV/AIDS Prevention, Program Evaluation Research Branch, for their support and review comments.

This manual was developed by David Napp, Practical Applications of Public Health, under contract to the CDC (contract # 66947) to provide technical assistance and support for implementation of the Guidance.
