This Funding Opportunity Announcement (FOA) supports statewide implementation of cross-cutting approaches to promote health and prevent and control chronic diseases and their risk factors and includes four programs:

  1. Diabetes
  2. Heart Disease and Stroke Prevention
  3. Nutrition, Physical Activity, and Obesity
  4. School Health

CDC announces the availability of RFA-1308: Promoting Adolescent Health Through School-Based HIV/STD Prevention and School-Based Surveillance [pdf 771K]. This Funding Opportunity Announcement (FOA) supports efforts to:

  • conduct school-based surveillance on youth risk behaviors and school health policies and practices;
  • implement school-based programs and practices designed to reduce HIV infection and other STDs among adolescents; and
  • reduce disparities in HIV infection and other STDs among specific adolescent populations.
