REACH U.S. Communities

CDC's Racial and Ethnic Approaches to Community Health Across the United States (REACH U.S.) communities serve as the cornerstone of CDC’s efforts to eliminate racial and ethnic health disparities in the following groups: African Americans, American Indians, Hispanics/Latinos, Asian Americans, Alaska Natives, and Pacific Islanders. REACH U.S. uses community-based participatory approaches that address social determinants of health through policy, systems, and environmental changes. Communities implement and evaluate innovative strategies for improving health in communities, health care settings, schools, and work sites.

REACH U.S. funds 40 communities in two categories:

• Centers of Excellence in the Elimination of Disparities (CEEDs) – These 18 national expert centers implement, coordinate, refine, and disseminate programmatic activities. CEEDs also provide pilot funding, support, local training, and guidance to projects to encourage the initiation or enhancement of work toward the elimination of health disparities.
• Action Communities (ACs) – These 22 communities carry out and evaluate successful practice- and evidence-based activities and programs that positively affect communities and cultural and environmental influences related to health disparities.
