List of ACHIEVE Communities

Ten initial communities were selected in January 2008. In March 2009, an additional 43 communities were selected to join ACHIEVE. The list below of the 53 current ACHIEVE communities shows the community funded, the agency awardee in parentheses, and CDC's national partner providing technical assistance to the community. ACHIEVE national partner names are abbreviated as follows: National Association of Chronic Disease Directors (NACDD), National Association of County and City Health Officials (NACCHO), National Recreation and Park Association (NRPA), and YMCA of the USA (Y-USA).
