Eligibility and Application Information

Eligibility

You must be a

• Medical or veterinary student in your 3rd year when you apply
• Available for at least 6 weeks during your 4th year

Education

You must be enrolled in a school accredited by one of the following:

• Liaison Committee on Medical Education
• American Osteopathic Association
• American Veterinary Medical Association

Citizenship

You must be a U.S. citizen or permanent resident.

 

Program Timeline

Fall Semester (June–December)

January 15, 2012	Application period opens
March 30, 2012	Application deadline (midnight, EDT) Postmark date for supporting documents
July 15, 2012	Selected applicants notified (Earlier if June or July date assigned)
June 2012	The program begins

Spring Semester (January–May)

January 15, 2012	Application period opens
May 30, 2012	Application deadline (midnight, EDT) Postmark date for supporting documents
September 15, 2012	Selected applicants notified
January 2013	The program begins

 

Application Process

The application period for the Spring and Fall semesters has closed.

 

Supporting Documents

Supporting documents must be postmarked by the deadline.

 

Letters of Recommendation

• Two letters of recommendation are required
• Select individuals who know you professionally and with whom you have recently worked
• At least 1 letter must be from a faculty member of your school
• Provide them with a copy of the Instructions for Letters of Recommendation  (2 pages, 83 KB)
• Letters must be mailed directly to the Epidemiology Elective Program by the deadline

 

Academic Endorsement

• Download the Academic Endorsement  (1 page, 28 KB)
• Have the form completed and signed by the student affairs office at your medical or veterinary school
• Mail the form to the CDC Epidemiology Elective Program by the deadline
