Eligibility and Application Information
Eligibility
You must be a
- Medical or veterinary student in your 3rd year when you apply
- Available for at least 6 weeks during your 4th year
Education
You must be enrolled in a school accredited by one of the following:
- Liaison Committee on Medical Education
- American Osteopathic Association
- American Veterinary Medical Association
Citizenship
You must be a U.S. citizen or permanent resident.

Application Process
The application period for the Spring and Fall semesters has closed.
Supporting Documents
Supporting documents must be postmarked by the deadline.
Letters of Recommendation
- Two letters of recommendation are required
- Select individuals who know you professionally and with whom you have recently worked
- At least 1 letter must be from a faculty member of your school
- Provide them with a copy of the Instructions for Letters of Recommendation (2 pages, 83 KB)
- Letters must be mailed directly to the Epidemiology Elective Program by the deadline
Academic Endorsement
- Download the Academic Endorsement (1 page, 28 KB)
- Have the form completed and signed by the student affairs office at your medical or veterinary school
- Mail the form to the CDC Epidemiology Elective Program by the deadline
