You must be a

Apply online between January 15 and May 30. Mail supporting documents to the CDC Epidemiology Elective Program office.

Supporting documents must be postmarked by the deadline for your chosen semester. Faxed or e-mailed documents will not be accepted.

Two letters of recommendation are required
Select individuals who know you professionally and with whom you have recently worked
At least 1 letter must be from a faculty member of your school
Provide them with a copy of the Instructions for Letters of Recommendation (2 pages, 83 KB)
Letters must be mailed directly to the Epidemiology Elective Program by the deadline

Have the form completed and signed by the student affairs office at your medical or veterinary school
Mail the form to the CDC Epidemiology Elective Program by the deadline
