Eligibility and Application Information
Eligibility
You must be a
- Medical or veterinary student in your 3rd year when you apply.
- Available for at least 6 weeks during your 4th year.
Education
You must be enrolled in a school accredited by one of the following:
- Liaison Committee on Medical Education.
- American Osteopathic Association.
- American Veterinary Medical Association.
Citizenship
You must be a U.S. citizen or permanent resident.

Application Process
Fall Semester: The application period for the Fall semester has closed.
Spring Semester: Apply online for the Spring semester. Additional supporting documents are required.
Supporting Documents
Supporting documents must be postmarked by the deadline for your chosen semester. Faxed or e-mailed documents will not be accepted.
Letters of Recommendation
- Two letters of recommendation are required.
- Select individuals who know you professionally and with whom you have recently worked.
- At least 1 letter must be from a faculty member of your school.
- Provide them with a copy of the Letter of Recommendation
(2 pages, 83 KB). - Letters must be mailed directly to the Epidemiology Elective Program by the deadline.
Academic Endorsement
- Download the Academic Endorsement
(1 page, 28 KB). - Have the form completed and signed by the student affairs office at your medical or veterinary school.
- Mail the form to the CDC Epidemiology Elective Program by the deadline.
