State-Based Diabetes Prevention & Control Programs
Wyoming
Public Awareness, Prevention and Quality Improvement
The Wyoming Diabetes Prevention and Control Program has operated since 1994
through funding provided by the Centers for Disease Control and Prevention.
The challenges of diabetes and the risk of developing the disease are growing
statewide; therefore, addressing these issues is a public health responsibility.
Second, it focuses on moving forward with coordination and implementation of
programs and projects that tackle access to care issues, providing professional
and public education, promoting quality improvement for providers, promoting
health communication messages, and creating synergistic partnerships among provider
systems.
Collects, Analyzes and Distribute Diabetes Data
The Program focuses on defining the burden of diabetes by
assessing prevalence and incidence. The program collects, analyzes and distributes
diabetes data from the following sources:
- Behavioral Risk Factor Surveillance System estimates prevalence and examine
trend statewide and county-specific data to support planning at both levels;
- Wyoming Vital Records examines the number of diabetes-related deaths;
- Wyoming Hospital Discharge Data examines the number of hospital discharges
associated with diabetes;
- End-State Renal Disease (ESRD) Data tacks new cases of ESRD and patients
undergoing dialysis in Wyoming;
- Indian Health Services Diabetes Registry tracks individuals that use the
Wind River Indian Health Service clinic;
- Wyoming Medicaid Claims data provides information on the number and costs
of claims filed by persons with diabetes;
- Medicare Quality Improvement Organization provides Medicare claims data
on persons with diabetes; and
- Diabetes Quality Care Monitoring System monitors diabetes care for individual
patients and tracks care for the overall practice of health care providers
and educators, for the purpose of quality improvement initiatives.
Promotes Community Partnerships
The Diabetes Prevention and Control Program provides mini grants to community
based-projects who have demonstrated sustained activities in the area of:
- Education and training of diabetes educators and other health care providers
who work with individuals with diabetes;
- Development of information materials – pamphlets, brochures, posters, video
and audiotapes, etc.;
- Implementation of innovative/creative public awareness campaigns;
- Increase of physical activity levels and nutrition awareness among residents;
- Educational programs on diabetes self-management;
- Programs designed to provide social and emotional support to persons with
diabetes and their families;
- Increase of opportunities for individuals to assess their risk for diabetes
and increasing participation in screening for non-insulin dependent diabetes
among high risk groups; and
- Development of focused programs aimed at reducing health disparities for
high-risk populations with respect to diabetes prevention and control.
